Preplan for Services

You are welcome to Pre-Plan with or without Pre-Payment. Either way, simply use these secure forms we have provided below to submit your information to us. If you prefer, you may also set up an appointment with us in person at our office in Bellevue to meet with a Funeral Director and purchase your prepaid funeral (burial or cremation) services. Please feel free to call us with any questions at 425-641-6100.

Preparing Before Death

1. Complete Online Death Certificate Worksheet. We will need this information for the Vital Statistics.

2. Complete Authorization for Cremation and Disposition form. This form authorizes us to perform the cremation(Washington State Law allows this form to be completed by the person who is to be cremated or buried, or their Power of Attorney. If you are durable power of attorney, please complete and sign prior to the passing of the client and submit to us a copy of your Power of Attorney paperwork for approval along with the authorization for cremation form.)

3. Please email all documents to info@cascadememorial.com or fax to 425-641-2956 or to set up your wishes.

Questions

Is Pre-Payment required?
No, while pre-payment is preferred by many, it is not required. You may certainly pre-plan for your final wishes without pre-paying for our services by completing the steps above.

Is pre-payment available?
Yes, you may sign a contract and pay for the preplanned arrangements ahead of time. Please note there is an additional recording fee to set up a guaranteed prepaid trust.

Does Pre-Payment guarantee the price?
Yes, when you pre-pay for your service, you lock in today's prices and we guarantee that the price you paid for your services will be honored at the time of use.
